Please note: this form must also be completed by the pension partner in order to permit pre-November 1, 2006 LIF owners to withdraw up to 50% of their account balance. The Option One check box must be selected, and the pension partner must complete the Certification on page 3. A copy of the waiver form should be retained for the account file.
